 Job Description

In Ft. Worth, Texas: The Texas Association of Community Health Centers is seeking a Bilingual Spanish Licensed Clinical Social Worker to provide psychosocial diagnostic assessments by conducting interviews of patients and their families / significant others (if appropriate), contacting outpatients service providers, and investigating histories through contacting prior agencies, schools, court personnel, and other sources of collateral information in order to identify strengths and problems, and develop an individual service plan  (ISP) which maximizes patient functioning in their community.

  • zing a collaborative documentUtiliation approach, completes a comprehensive social determinants of health assessment which is maintained in the patient’s electronic health record (EHR) within five days of initial contact with the patient according to established center’s Quality Improvement Plan (QIP) workflow.
  • Coordinates the development of individual service plans (ISP) for patients on caseload. Ensures that the primary care provider team inputs are coordinated with ISP development according to established Texas Department of State Health Services (DSHS) Clinical Management Behavioral Health System (CMBHS) criteria and timelines.
  • Provides clinical case management for cases assigned to ensure timely and effective delivery and management of services identified on the ISP.
  • Provides patients with self-management skills and educational information needed so they can be full participants in their own treatment and recovery.
  • Provides highly skilled clinical social work services to patients and their families, utilizing a wide array of intervention modalities including Cognitive Behavioral Treatment (CBT), Motivational Interviewing, individual, group, and family therapy for the purposes of changing maladaptive behavior, enhancing client capacity to cope with symptoms of mental health challenges and / or substance abuse, completing a relapse prevention plan with patients who are in remission, and improving communication and interactions between the patient and his or her primary support system.

 Job Requirements

Qualifications:

  • Demonstrated knowledge of the principles and practices of psychiatric and basic social work practices, including but not limited to, integrative behavioral health, cognitive behavioral and solution focused therapies, collaborative documentation, substance abuse treatment, psychotherapies, psychological, and emotional growth, and development.
  • Familiarity with the types and applications of emergency medical procedures, such as CPR.
  • Basic uses of diagnostic procedures employed in social work practice.
  • Knowledge of motivational interviewing and problem-solving techniques.
  • Ability to follow and give clear, precise, oral, and written instructions.
  • Capacity to participate in a multi-disciplinary primary care health care team.
  • Skilled in preparing, implementing, and monitoring ISP’s.
  • At least three years of full-time, or equivalent part-time, professional experience as a clinical social worker after earning a master’s degree in social work.  Based on assignment to second-level supervisory positions, at least one year of experience must have been in a supervisory capacity. 
  • A Master's or higher degree in social work, Licensed Professional of the Healing Arts (LPHA) is required. 
  • Licensure as a Licensed Independent Clinical Social Worker or Licensed Practitioner of the Healing Arts (LPHA) by the Texas State Board of Social Worker /Professional Counselors Examiners is required.

Our facility has served the Fort Worth community since 2006, caring for a predominantly Hispanic, low income, uninsured and under-insured population. We offer pediatric and family practice medical services and contract with an outside dental provider for dental services. Our health center partners with several agencies in the area to ensure all patient needs are met. Located in Tarrant county, our service area is designated as a HPSA/MUA.
