Practice Setting:

The Health Center offers an integrated primary care, behavioral health, oral health, Substance Use Disorder and HIV prevention and treatment 'package' in multiple sites in El Paso and Hudspeth Counties.  Counselors work with primary care providers, care managers, and supporting psychiatrists in individual and family care, and use a hybrid model of virtual and physical encounters

 Job Description

In El Paso, TX: Texas Association of Community Health Centers is seeking a Full-Time School Based Bilingual (Spanish and English) Licensed Clinical Social Worker or Licensed Professional Counselor to provide direct clinical services to individual, couples and families and to perform basic administrative tasks in support of service provision.
 

Job Overview:

  • Assesses individual client needs; develops and implements individualized treatment programming in a school based setting
  • Monitors individual client progress toward specific goals and objectives; participates in coordinating depression care for patients in the Integrated Behavioral Health program
  • Utilizes caseload management systems; provides psycho social services; conducts individual, child, family, and group counseling sessions in accordance with professional standards; monitors depressive symptoms and response to medication and/or psychotherapy using a structured instrument (e.g., the PHQ-9, and OASIS);
  • Works closely with the Primary care provider; Psychiatrists, and Clinical Care Manager to revise the treatment plan when patients are not improving. Coordinates psycho social services delivery with appropriate agencies within the community; and performs other duties as assigned.

 Qualifications:
•    Texas Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW)
•    Minimum two years casework/case management or related experience.
•    Computer proficiencies in Microsoft Office Word, Excel
•    Reliable transportation and flexibility to travel for up to three days.
•    Bilingual — Fluent in both English/Spanish, reading, writing, and speaking.
•    Capacity to work as part of a team.
•    Capacity to work with families in a confidential, respectful, caring, professional manner


 Client Overview

Our facility is a non-profit health center with three sites in El Paso. The clinics provide a full spectrum of primary care services, including sliding fee scale clinic visits, health education and outreach, and low cost medications. In addition, our center operates a licensed Early Childhood Development Center, Roots and Wings, a transition living center for single parent families, afterschool programs in East Central and Northeast El Paso, a thrift shop and food buying cooperative, manages an apartment complex for transitional families needing housing assistance, and coordinates with WIC, family planning, adult education providers and a community credit union to provide those services in its facilities. -The facility sees all ages from infant to geriatric.
