 Job Description
Provides healthcare services to patients under direction and responsibility of Medical Director by performing the following duties as determined by the credentialing and privileging of CHCACT. Evaluate and treat any patient presenting to any registered patient of the Community Health Centers of South Central Texas, Inc., following guidelines as set by the Medical Director. Obtains medical and developmental history and records findings in patient medical records. Performs complete history and physical examinations. Orders, interprets and reviews basic lab and radiographic studies (x-ray mammogram) necessary for establishing diagnosis and the appropriate therapy. Promptly completes an encounter form for each patient visit. Recognizes common illnesses and injuries and consults with or refers to physician when necessary. Prescribes selected diets and physical treatment as appropriate. Provides follow-up care and health instruction through telephone communications and scheduled clinic appointments. Will provide a contractually set minimum number primary care encounters for the contract year. Instructs, educates and counsels patients individually or in groups. Performs family planning physicals and tests of cure. May perform minor surgical procedures in clinic as determined by prior credentialing. Serve on appropriate health center committees to include compliance Improvement/Risk Management, Providers, Pharmacy, and Medical Staff. Occasionally provides primary health care services and/or coverage to registered CHCSCT patients at off-site locations such as CHCSCT stationary or mobile satellite clinics, CHCSCT sponsored Health Fairs, school clinics, immunization clinics. Nursing homes, senior citizens center, community churches, various public locations, and hospitals located within the CHCSCT service area. Performs physicals and/or histories of registered patients who are hospitalized at local hospitals. Provides referrals to MD/DO Sub-Specialists, Dentists, and Behavioral Health Department. Submits agency forms and documentation in a timely manner. (Credentialing and re-credentialing forms, improvement activities, response to memorandums, travel reconciliation, supply requests, leave requests, monthly in-service). Actively participates in agency performance improvement initiatives such as chronic care management. Positively contributes to staff morale and corporate culture. Ensures that work assignments and information gathered from day to day is not shared with anyone and protects all patient and agency information. Is knowledgeable of and maintains HIPPA standards of privacy and confidentiality. Supports the CHCSCT mission, vision, and values statement. Perform other duties as assigned and which may be in keeping with the position.
