 Job Description

In Trinity, TX: Texas Association of Community Health Centers is seeking a Full-Time Family Nurse Practitioner/Physician Assistant who is responsible for providing primary and preventive health care in a warm and friendly community healthcare setting.

Job Overview:

  • Prescribe medications and make recommendations for other forms of treatment
  • Maintains medical expertise and competency as well as demonstrates a thorough knowledge of procedures to deliver quality primary medical care
  • Examines, diagnoses treats and/ or refers patients for appropriate specialty care services
  • Maintains a problem-oriented health record for each patient seen in accordance with the health center's established information management policies and procedures
  • Participates in the review and development of patient care policies, including health maintenance schedules and continuity of care

 Job Requirements

Qualifications:

  • An unrestricted License to practice as a Family Nurse Practitioner/Physician Assistant with prescriptive authority in the State of Texas
  • 1 year of post-training experience required 
  • Master’s degree (or equivalent) and Board Certification in specialty
  • Proficiency in Computer Skills and Electronic Health Records

Bilingual (Spanish & English) Preferred

Trinity was founded in 1872 on land purchased from the New York and Texas Land Company. The town was a railroad station on the Houston and Great Northern Railroad.

The town was originally named Trinity Station after the Trinity River. The name of the town was later changed to Trinity City, then to Trinity. The town was incorporated in 1910.
