Description |
|
Serves as a member of the Palliative Care Team, providing consultative and follow-up care upon referral to patients and families. Practices in collaboration with the Palliative Care Medical Director. Coordinates care and services for patients and families across the continuum of care. Acts as a resource to staff and families regarding palliative/end of life care. Promotes palliative care within the organization and the larger community. This is a 40 hour per week position M-F with no weekends or holidays. Master degree -Nurse Practitioner with a Maine license, prescriptive ability in the state of Maine with a current DEA certificate. The ideal candidate will have a minimum of 3 years of experience, preferably in palliativve care, oncology and or hospice. Must have excellent assessment, diagnosis and treatment skills, excellent communication skills and be self directed, motivated, and highly organized.

Our Medical Center
Anchoring our healthcare system is a 287-bed medical center with campuses in Augusta and Waterville.
Services include emergency and critical care, a full range of inpatient and outpatient surgery, diagnostics, substance abuse and mental health services, cancer care, maternity newborn and child health programs.
The Jackman Region Health Center in northern Somerset County, which is part of the medical center, provides physician office services and an 18-bed nursing home.
