Description |
In Uvalde, TX: Texas Association of Community Health Centers is seeking a full-time Licensed Professional Counselor/ Licensed Clinical Social Worker who is responsible for evaluating and providing mental health and substance abuse education and treatment for patients with identified behavioral health conditions and/or substance abuse diagnoses.
Job Overview:
- Admits new patients to behavioral health by reviewing records and conducting orientation
- Determines patients' requirements by completing intake interviews; determining therapeutic medical, psycho-social and psychiatric needs; reviewing behavioral health and substance abuse evaluations, treatment objectives and plans
- Educates patients and/or their family about diagnosis, plan of care, ordered screening, medications, etc.
- Assists and supports patients to follow through on prescribed mental health, medical and substance abuse interventions and diagnostic screening
- Promotes patients' self-management and empowers patients/families to achieve maximum levels fo wellness, independence and self-care
- Provides individual, family and group psychotherapy and/or substance abuse counseling under the diretion of the psychologist
- Establishes treatment programs by setting schedules and routines; coordinating service delivery; and arranging resources, including transportation
- Monitors cases by observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support and outreach
- Refers to higher level of care when appropriate, with facilitation of inpatient or residential psychiatric or substance abuse treatment admissions

Qualifications:
- LCSW/LPC license in the State of Texas
- 2 years of post-graduate training experience required
- Familiarity with and sensitivity to multicultural groups, their cultural differences and family customs
- Must be CPR certified
Bilingual in English & Spanish preferred!
