In 1984 The Darboy Joint Sanitary District was completed, and paved the way for construction of homes and businesses. The area has experienced steady growth in the last 20 years and has built two new elementary schools in 1996 and 2006 respectively. Kimberly High School was also expanded recently to meet the needs of this growing community. Today, Darboy has a thriving commercial district, made up of many high profile businesses, including 7 large stores, 8 banks and countless shops, convenience stores, and other small businesses. |

| Income Details: | This is an employed position with a flexible two-year salary guarantee based on a four, four and one-half or five day work week. After that period of time, the physician will move to an incentive compensation plan. There is an option to go on the compensation plan after one year, if the physician meets financial targets. |

ThedaCare, Inc., a highly successful northeast Wisconson integrated health system, seeks a BE/BC Family Medicine physician for our Darboy location. This new, 23,000 sq-ft, state-of-the art facility, is scheduled to open in December 2009. This position will partner with a dedicated and innovative medical staff including midlevel providers. This opportunity will allow the physician the chance to work in setting where growth and change are encouraged and expected. As part of ThedaCare's new delivery model, providers will offer same-day appointments and a plan of care for the patient before they leave. In addition, patients will know their laboratory results during their visit! The practice will consist of primary care, pediatrics, physical therapy and diagnostic and lab services. Members of the group currently perform vasectomies, colposcopies, lumps and bumps, and joint injections. Other procedures are welcome! The group shares call with another family medicine group. Call will be 1:14 weekdays; 1:13 weekends. OB call is currently at 1:4. |

| APPLETON MEDICAL CENTER is highly regarded as a top area medical facility. This 160 bed hospital is breaking new ground and records with its key programs and services. Named four times as a Solucient "Top 100 Hospital". The Emergency Department is staffed by board certified physicians. The Appleton Heart Institute has been a leader for more than 20 years in the prevention, diagnosis equipment, medical and surgical treatments and rehabilitation, and currently performs more than 300 open-heart procedures annually. The ThedaCare Cancer Institute and the Martha Siekman Cancer Center offer excellent oncology services for area conacer patients who require inpatient or out patient therapy and ongoing support services. The Groth Surgery Center, which opened in 1999, is a state-of-the-art outpatient surgery center completed with seven surgical suites, four procedure rooms, and a cystoscopy room.

| Practice Description : | This is a community-based family medicine practice; OB is optional. The physicians may choose between using the hospitalist programs at Appleton Medical Center and Theda Clark Medical Center or they may follow their own hospital patients. Physicians may choose a four, four and one half or five-day work week. Physicians also enjoy working with a fully integrated Electronic Health Record. Ancillary services include a full-service lab, digital x-ray and mammography. |
