| Income Details: | This is an employed position with a flexible two-year salary guarantee based on a four, four and one-half or five day work week. After that period of time, the physician will move to an incentive compensation plan. There is an option to on to the compensation plan after one year if the physician meets financial targets. |

ThedaCare, Inc., an integrated healthcare system in Northeast Wisconsin, seeks a BE/BC Family Medicine Physician for its Black Creek location. This is the ideal position for a Physician who would like to practice independently while being part of a larger call group. The call is shared with another ThedaCare Family Medicine site, making call only 1 weekday/month, and 1:17 weekends. 2 half clinic days are part of the wknd. call rotation. OB is optional and the Hospitalist Program can be utilized for hospital admissions. This Physician will share the practice with 2 part-time NP's. Black Creek is a small quaint community of approximately 1,200 people, located 15 mi. North of the Fox Cities which include; Appleton, Neenah, and Menasha. This area offers the charm, safety, and convenience of small town life while being a stones throw away from a Metro area. On Main St. in Black Creek you will find; our clinic, a grocery store, several other shops, as well as a library. An added bonus is that a network of local growers get together and offer a huge farmers' market right downtown. Down the road you have access to the Fox Cities. The population of the Fox Cities and surrounding areas reaches 1 million within a 60mi. radius. It is easy to get from here to there, generally within 20 minutes or less.The area economy is healthy, producing low unemployment and sufficient revenues to ensure that area residents enjoy the municipal necessities that make life safe and fun, such as; plentiful parks, exceptionally strong school systems, and fire & police protection. There is a rich selection of culture, recreation, as well as churches, synagogues, and temples to accommodate just about every religious faith. There are 3 major shopping malls, including the famous Fox River Mall in Appleton. |

| Practice Description : | Small Town Friendly with Big City "Like" Facility:ThedaCare Physicians Black Creek is a 2,600 sq-ft facility with a fully integrated EMR and digital PACS system to view radiologic films. The facility has 4 exam rooms, 1 procedure room, as well as a central nursing station and dedicated physician office. Practice Opportunities:The current physician, who is relocating within the organization, performs minor office procedures and sees newborns at the local hospital. Leadership and volunteer opportunities are available within ThedaCare and the local community, including but not limited to: disease management, volunteering as the attending at local family medicine residency program, precepting medical and nurse practitioner students, medical directorship with local schools, and providing local nursing home coverage . |
