Outreach

Our care and concern for a patient goes beyond just their physical health. We understand if you can’t afford health care, can’t pay for your basic utilities or don’t have enough to feed your children, that your personal world will be filled with stress which can lead to medical problems.

While we can’t help with all problems, our Case Managers attempt to find any resource that can provide our patients. Many times the help needed is just temporary, but it can make a big difference in a persons life. Our Outreach department is the heart and soul of our organization.

Counseling

Our goal is to create a healthy mind, body and spirit. Our Behavioral Health services recognize that we as individuals, at times, encounter circumstances in our lives that may be challenging.

We help to re-direct thoughts and behaviors in a positive direction to enable our patients to achieve their goals. Counseling sessions are offered on an individual or family basis and group sessions are also available for patient convenience.

340B Pharmacy

The 340B program was established to provide qualifying patients with affordable prescription drugs. The program is available to certain types of safety net clinics/providers. It allows these providers to stretch the limited federal resources available to them and to offer a comprehensive list of prescriptions at significantly reduced prices.
